I don't think it's stimulation from the clit. I get this sensation when my g-spot gets stimulated and yes, it can be stimulated with a penis. Just make sure she goes pee before you start sex and when she feels this tell her it's normal and to try and relax because if you keep doing it, it won't feel like she has to pee anymore.
